Download, print, and complete the NYSMF Application Forms. ALL FORMS ARE REQUIRED FROM ALL APPLICANTS.

Includes the following forms:

  • Application Form
  • Teacher Recommendation Form
    To be filled out by your teacher, conductor, lesson teacher, etc. (May be sent separately)
  • Permission Form
    Provides your parent or guardian's permission to participate in various recreational activities, as well as listing the basic NYSMF Code of Conduct.
  • Financial Aid/Scholarship Form
    Required for consideration for NYSMF Scholarships or need-based financial aid.
    • SCHOLARSHIP APPLICANTS MUST INCLUDE A DEMO TAPE THAT INCLUDES 1-2 movements of concertos (or short pieces) of contrasting styles
  • Health Form Checklist
  • Health Examination Form
  • Medication Form
  • Meningitis Information Sheet
  • Meningitis Vaccination Response Form
